Understanding IRMAA
IRMAA (Income-Related Monthly Adjustment Amount) is an additional premium paid by higher-income Medicare beneficiaries.
Medicare reviews your federal income tax return from two years earlier. For 2026 Medicare premiums, Social Security generally uses your 2024 Modified Adjusted Gross Income (MAGI) to determine whether an adjustment applies.
IRMAA can increase the cost of:
- Medicare Part B
- Medicare Part D (including Medicare Advantage plans with prescription drug coverage)
Life Changes May Lower Your IRMAA
If your income has decreased because of one of these qualifying events, you may be able to request a lower premium:
✓ Retirement
✓ Marriage or divorce
✓ Death of a spouse
✓ Loss of pension income
✓ Loss of income-producing property
✓ Employer settlement changes
The request is made by filing Form SSA-44 with Social Security.
